Expanded Duties Programs
The application requirements for Board approval of an Expanded Duties Program are as follows and can be found in Rule 64B5-16.002, F.A.C.:
The application must include:
- A completed application form, signed and dated by the Program/Course Director.
- Documentation of training and experience of faculty members qualified to teach specified subject areas
- Student/Teacher ratio
- Detailed syllabus of the course or program
- Documentation that the course includes instruction in the following subjects
- Sterilization and Disinfection Procedures as stated in Rule 64B5-25, F.A.C.
- Instruction in the Dental Practice Act and Administrative Code as it relates to dental auxiliaries
- Documentation of clinical or written examination which adequately tests competency in each required subject area
- A sample certificate of completion
- Applications may be submitted by mail or email and must include all required items listed in Rule 64B5-16.002, F.A.C.

Mail: Applications may be submitted by mail to the Florida Board of Dentistry at 4052 Bald Cypress Way, Bin-C-04, Tallahassee, FL, 32399. - Application will be reviewed upon receipt and correspondence regarding the status will be sent to the contact person listed on the application. Please allow up to 10 days for initial review of new applications.
- When all required items have been reviewed by the Board and deemed complete, a letter of approval will be sent to the contact person listed on the application. Please allow up to 7 days for the approval process to be completed.
- Once approved, the program information will be listed on the List of Board Approved Expanded Duty/Dental Radiography Programs available at Index of /pub/hcpr/Dentistry/List of Board Approved Expanded Duty_Dental Radiography Programs
There is no fee for this application.
