Florida passed House Bill 975 following the 2024 legislative session, which requires this profession to complete electronic fingerprinting. Your application cannot be approved until this requirement, along with all other licensure criteria, has been met. For more information on background screening, visit MQA's Background Screening website.

Requirements

The board office staff will process the renewal of a non-profit corporation permit upon receipt of:

  1. Completed Non-Profit Corporation Permit Application
  2. Proof current CPR at the basic life support level

Renewal notices are not sent to the non-profit corporation.  Failure to renew an active permit by the expiration date will render the permit expired without any further action by the board.  A new application and supporting documents are required in the event a permit expires.

Continuing Education

There is no continuing education requirement for a non-Florida licensed dentist employed by a non-profit corporation.

Process

An officer of the non-profit corporation is required to RENEW BY MAIL by submitting the Non-Profit Corporation Permit  Application and proof of current CPR at the basic life support level for each non-Florida licensed dentist at the facility prior to the expiration date to the following address:

Department of Health
Board of Dentistry
4052 Bald Cypress Way, #C-04
Tallahassee, FL 32399-3258

Renewal notices are not sent to the non-profit corporation.
